SAHS SENIOR RECOMMENDATION REQUEST FORM
During the senior year of high school, a student may need to obtain a letter of recommendation from their school counselor. Students are asked to fill out the form below to give their counselor key information so that the appropriate recommendation letter can be written for a job, college application, scholarship application, or other opportunity. Counselors do ask that the request form be submitted at least two weeks prior to the date that the letter is needed. Students will need to log in using their email account to access and complete the form.
